Refunds will be provided, on request, if the order is not timely filled. See "Fulfillment of Your Order", set forth below.

| FULFILLMENT OF YOUR ORDER |
| Within 90 days of receiving your order and payment, HEARTFLEX, Inc. will ship the product(s) you ordered, or if all or some of the ordered product(s) are unavailable for any reason, will ship the available product(s) and for product(s) that are not available: (1) mail a refund or, if payment was made by means of a transfer from an account, (a) credit the account in the amount of the debit, or (b) if a third party is the creditor, issue a credit memorandum to the third party, who shall promptly |

credit the account in the full amount of the debit, or (2) send the buyer a letter or other written notice (a) advising the buyer of the duration of an expected delay expressed as a specific number of days or weeks and (b) offer to make a refund, in accordance with paragraph (1), within one week if the buyer so requests. Orders should be made by calling our toll-free number of 1-800-920-8118. During the telephone call, we collect visitor's contact information (like name, address, phone number, and email addresses), unique identifiers (like driver's license numbers for check purchases), and financial information (like customer's account or credit card numbers).
Contact information from the order form is used to send orders to our customers. The customers' contact information is used to get in touch with the customer when necessary. Financial information collected is used to bill the customer for products and services. Unique identifiers (such as driver's license numbers) are collected to enforce payment of insufficient checks.
